After watching Torn and Frayed once more, I suspect that Naomi is actually Cas with a split personality.
we know that Cas arrived in Purgatory half-mad from Sam's Hellucinations, yet once in Purgatory he seemed to be his old self, so perhaps he 'performed a lobotomy' on himself (we see these visuals in this episode) seperating 'Cas' from his angel self, whom he now personifies as Naomi
there is never an explanation of where in heaven Naomi is, or what her purpose may be, (we only see her in Cas' head) but we know that Cas needed penance for what he had done (season seven).
Cas tells Dean he cannot escape from Purgatory, but this might not be the case, he might have - in his guise of Naomi - found Benny and directed him to Dean, making a way for Dean to leave.
Yet, if Cas discovered that Dean were in danger from Crowley, he might leave Purgatory, telling himself the force was this other half, his more angelic half..
Why a female- who is the same height as Cas, with dark hair and blue eyes - Anna was once his commander, Rachel his lieutenant, and Hester were all righteous angels.
Cas needs a firm hand, is lost without guidence, he has given himself over to the care of people (Dean in particular) and while conflicted, he goes along with Naomi's forceful commands.
Samandriel first appeals to Naomi for help, but Cas tells Dean that he heard Samadriel's call for help.
Samandriel confesses to Cas that he 'knows the truth' about Naomi - and Naomi tells Cas to kill Samandriel 'in self defense' in this case, to keep the secret that Naomi is Cas.
Naomi in scenes with Cas in 'heaven' is transparent.
